Please note: On pages 4 and 5 under "Reflections Checklist for Units", the time limitation for Dance Choreography should be three (3) minutes and not five minutes as printed; the time limitation for Musical Composition was omitted and should be five (5) minutes.

Instructions for personalizing and printing Certificate of
Participation:
1. When filling out your certificate, please center your cursor to the
middle of the line in which you are typing (for all fields).
2. You can fill in the student’s name, category, PTA name and date to
personalize for each student.
3. Save the document to your computer and print out the form on either parchment paper or card stock.
4. You can then hand sign each certificate.
