Membership Card Confirmation (Yellow Form)

Membership cards are being mailed to units who have returned their Annual Officers' Contact Information Sheet (Form A), paid their insurance and have no outstanding membership issues from last year. Traditionally, when the cards are received, presidents are asked to complete and return a Yellow Membership Reply Form indicating the number of cards received and provide NYS PTA with the address information for their Unit Membership Chairman. We encourage you to utilize the online form. By doing this, the process will be expedited and we will be able to provide better service. However, the hard copy of the reply form is still available should you need to use that method. Please be sure to submit your information using only one of these methods.
